Memorial Health Partners (MHP) is a Georgia-based Clinically Integrated Network (CIN) and Provider Network. Since 1996, MHP's comprehensive provider network has contracted with Memorial Health, third-party administrators, and fully-funded and self-funded employers. The network is comprised of more than 1,700 clinicians including physicians and advanced practice providers as well as 16 acute care hospitals across southeast Georgia and South Carolina who have come together in value-based care. MHP's board of directors is physician-led and professionally managed. The MHP team works collaboratively with the provider network and payers to ensure patients are managed in an efficient, quality-driven manner.
