Cardiac and Pulmonary Rehabilitation is a special program designed to help individuals who have suffered a heart attack or who are fighting cardiac or pulmonary disease. The program aims to help you recover faster and improve your quality of life.
The program: Incorporates a thorough medical evaluation, education, exercise and counseling services to meet individualized needs. Teaches participants about heart disease, pulmonary disease and how to cope with their physical and physiological symptoms.
Cardiac Rehab lasts up to 36 sessions, or three months, and includes monitored exercise to strengthen the heart, lungs and overall condition.
Pulmonary Rehab lasts up to 24 sessions, or six weeks, with the end goal of living an independent lifestyle even with a respiratory condition.
Education and nutrition counseling are included in both programs and is designed to assist participants in managing their condition and reducing the risk for future heart and lung problems.

Peripheral Artery Disease (PAD) is a vascular disease, caused by gradual build-up of plaque within the leg arteries. It may cause disabling symptoms, such as:
- Leg cramping while walking
- Leg pain while resting
- Nerve damage
- Skin breakdown
Supervised Exercise Therapy (SET) can help people suffering from PAD pain. The exercise therapy targets the muscles that hurt when you walk, such as the:
- Calf
- Thigh
- Buttock
The goals of SET for people with PAD are to:
- Increase functional capacity and walking distance
- Decrease symptoms
- Reduce risk factors for PAD, heart attack, and stroke

- Exercise sessions last 30 to 60 minutes or longer

Why Supervised Exercise Therapy (SET) for Peripheral Artery Disease (PAD)
Research has shown supervised exercise therapy (SET) to be an effective, minimally invasive method to alleviate the most common symptom associated with peripheral artery disease (PAD) - intermittent claudication (IC).
SET has been shown to be significantly more effective than unsupervised exercise, and could prevent the progression of PAD and Iower the risk of cardiovascular events that are prevalent in these patients.
SET has also been shown to perform at least as well as more invasive revascularization treatments that are covered by Medicare.
- Consist of sessions lasting 30-60 minutes comprising a therapeutic exercise training program for PAD in patients
- Must have a face-to-face visit with the physician responsible for PAD treatment
- SET is non-covered for members with contraindications to exercise as determined by their primary physician
